Premature ejaculation, or PE, occurs during sex when orgasm takes place sooner than a couple would like. It is important to realize that that the average time before release of sperm during sex is about 7 minutes. You should not have unrealistic expectations of lasting longer than this time. Severe PE occurs when orgasm takes place in less than a minute.
It is important to separate PE into primary and secondary PE. Primary premature ejaculation occurs when the problem has been present throughout the man’s life. Secondary PE occurs when the problem started recently.
Primary PE is more difficult to treat. It may be due to several different factors. Also, it may simply reflect how your penis is “wired”. In other words primary PE may simply be the result of how the nerves in your penis are structured. This situation cannot be easily changed. Secondary PE, or recent onset PE, is more easily treated.

The exact cause of PE is not currently known. Serotonin is a chemical in the brain that is believed to play an important role. High levels of serotonin in the brain increase the time to ejaculation, while low levels shorten the time to ejaculation – leading to PE.
Psychological issues may also be involved in the cause of PE. These include:
- Unrealistic expectations about sexual performance
- Lack of confidence
- Relationship issues
- Temporary depression
- History of sexual repression
There are three main PE treatments used today. These include psychological therapy, behavioral therapy, and medical therapy.
Psychological therapy is an effective way to address negative emotions or feelings that may cause issues in sexual relationships. The goal of this therapy is to identify the source of the problems and to find the solutions. Besides improving PE, psychotherapy may also help couples grow closer. Psychotherapy may improve overall sexual performance by reducing stress and anxiety during sex. A psychologist or psychiatrist may effectively treat PE using this approach.
Dr. Lazare has created several YouTube videos that use behavioral therapy to treat premature ejaculation. The strategy of behavioral therapy is to heighten your awareness of your level of sexual excitement during sex. Using several techniques, you learn to control your excitement level. You then learn how to lower your excitement level when you approach orgasm. This enables you to prolong the time before you ejaculate and to help you last longer during sex.
While there are no drugs that have been approved in the U.S. to treat PE, some medications and numbing creams or sprays have been shown to occasionally help men with this condition.
Dr. Lazare thinks that the best medications for PE are Viagra, Cialis and Levitra. They often delay the onset of orgasm. In addition, if you come too quickly, you can engage in a second round of sex. During this second attempt, you will probably last longer. Even better, these medications have minimal side-effects.
Dr. Lazare is not enthusiastic about using anti - depressants for the treatment of PE due to their side effects. The medications most commonly used for this purpose are the SSRI’s or selective serotonin reuptake inhibitors. Two examples are Zoloft and Prozac. Although these medicines work well to delay orgasm, they produce drowsiness as a side effect. This drowsiness is a significant problem, and it can cause problems when you are driving or trying to work.
Numbing creams and sprays can be used on the head of the penis about 20-30 minutes before sexual activity. They should not be left on the exposed penis during vaginal intercourse as they may cause vaginal numbness. It is important to remove these medications from the penis before sex. The results of these numbing medicines are mixed.
